Voter Registration Affidavit
The Woody Creek Voter Registration Affidavit is required by our Bylaws for all members of the caucus who would like to vote, in person OR by absentee.
To vote by absentee ballot, you must first complete with Woody Creek Registration Affidavit. You can do this at any meeting you attend during the year, or print it online (below) and fill it out and bring it to any meeting. The purpose of this form is to encourage participation in the meetings, and members of the caucus must attend 1 meeting per year to vote.
You must include the date of the meeting attended on the form! It will be checked against our attendance lists from every meeting and verified by the DPC (District Planning Commission).
